Remarks: Food permit suspended (compliance conference scheduled). Fryer basket handles have an accumulation of grease on them. Metal portion of ice chest was rusting. Raw fish stored above cabbage and mac and cheese in 2 door refrigerator. Cooked hotdogs stored directly on open container of shelled eggs in refrigerator. Raw ham stored directly on container of cooked ribs in 2 door refrigerator. Raw sausage stored above open container of fish in freezer. Foods made in facility did not have date marks. Observed milk in 2 door refrigerator with expiration date of 1/29/14. Observed employee preparing food while not wearing a hair restraint. BBQ made previous day in facility was placed in steam table to reheat.

Remarks: Facility has changed name to Suwanee Pizza and Chicken. Observed employee touch cooked sweet potatoes with bare hands. At beginning of inspection, employee was sitting in kitchen at the back prep table eating a bowl of food. Container of raw beef stored behind potato wedges and tomato sauce in prep cooler, causing raw beef to be lifted over and across potatoes and sauce when removing from cooler. Sauce for chicken wings contains cooked carrots and onions and is potentially hazardous. Observed employee preparing food while wearing bracelets on both wrists. Observed employee preparing food without any form of hair restraint. Observed employee wearing gloves pick napkin off floor and continue preparing food.

Remarks: The permit has been reinstated due to having hot water restored to the facility. All sinks in the facility have hot water restored (lowest acquired temperature was 109°F). The owner has stated that he intends to make one of his employees his Certified Food Safety Manager.
